No. It does not generate naturally in the world; it must be crafted or cut from granite blocks.

What tool do I need to mine it?

Any pickaxe, wood tier or higher. Mining without a pickaxe destroys the slab with no drop.

Can two granite slabs form a full block?

Yes. Placing one slab on top of another in the same block space combines them into a full granite block visually, matching vanilla double-slab behavior.
