"A mysterious power emanates from the talisman..."

The cosmic talisman grants access to the Cosmic Altar in Zanaris, where players can craft cosmic runes at 27 Runecraft. The talisman can also be combined with a tiara to create a cosmic tiara for 40 Runecraft XP, freeing up an inventory slot for runecrafting trips.

A common drop from abyssal demons and dagannoths, and occasionally rewarded from random events.
